Path: Configuration>>PMS>>PIP>>PIP Template

Click on Add to create new PIP Template
Enter PIP template name Select Org master & Org details
Enter Question details and there Question order
Click on save
You can create multiple PIP Template by Organization master wise

In case already added click on search to view previous PIP Template
